I would like to take this opportunity to express our gratitude for the services that your company and employee's provide for Galichia Heart Hospital.  We are fortunate that companies support the quality services that we strive to excel in each day as we provide care to our clients.  Part of our quality service includes a commitment to providing services that meet and exceed our regulating agencies requirements.  With this we would like to ask for your help ensuring our compliance by providing us with information to ensure that you are maintaining health practices conducive to our care environment.  Please read the Hospital Orientation information, print off and sign the acknowledgement; print off and complete the Health Screening (we insist that every vendor provides a copy of their most recent TB test results), and view the policies on Health Screenings.  I have also included our Confidentiality form that must be completed prior to you coming to our facility along with the Vendor Expectations sheet. 

Please remember that when you visit our hospital, that you must sign in at the Admissions desk and then the admissions staff will announce your arrival to the department in which you are visiting.  We ask that the number of non-appointment meetings be limited.
